Here's how to join.

Instructions

Step 1. Fill out and submit the USPPG electronic registration form and submit it to us. Make sure all the required fields are completed and an email address is provided. Upon verification, you will receive an email confirmation from USPPG giving you a secure password to access the site fully.

Step 2. If you purchase your vaccines from GlaxoSmithKline, you must register with GSK to be included on their vaccine contract.  Click here to register with GSK. You will receive a notice from GSK confirming receipt of your application. After GSK verifies the DEA# associated with the practice you will receive a "Notice of Acceptance" from GSK. The entire GSK process takes approximately 2 - 3 days.
